Absolute Eosinophilia Due to Parasite, Resembling Eosinophilic Leukemoid Reaction. (A case report)

Abstract
There are many cases of absolute eosinophilia in peripheral blood of which the most
common are of allergic and parasitic origins. Among parasites trichinosis, visceral larva
migrans, sparganosis and gnathostomiasis are the most common causes eosinophilia.
The authors here presents a case of absolut eosinophilia (82%) with marked
leukocytosis (48,600/§§) due to a parasitic disease.
On April 8,1976, a 28-year-old Korean hausewife married to a U.S. army soldier was
admitted to Han Yang University Hospital. Prior to admission she had lived in the
U.S.A. for several months. She was admitted because of palpable subcutaneous masses
on the left breast, periumbilical and right inguinal area. Chest X-rays revealed a
pleuritis of unknown origin. A biopsy was taken near the umbilical area under the local
anesthesia. Paraffin sections showed a cystic structure lined by a rather thick wall
composed of macrophages, foreign body giant cells and numerous mature eosinophils.
The cystic lumen contained necrotic debris mixed with mature eosinophils and the
pericystic area also showed massive and heavy infiltration of eosinophils throughout.
There were no recognizable parasitic worms, larvae or eggs.
A parasitic origin was suspected, but there were no recongnizable parasite in multiple
sections.
